LifeWise Assurance Company Digital Privacy Policy

At LifeWise Assurance Company (“LifeWise,” “we”), protecting your privacy is important to us. This Digital Privacy Policy explains the types of personal information we collect through our website and mobile application (“mobile app”), how we use and share that information, the choices available to you, and the steps we take to safeguard your information.

Collection of personal information

Personal information is information that identifies you or relates to you.

Personal information collected through the website or mobile app

We collect personal information through the website and mobile app in two ways: (1) information you provide directly to us and (2) information automatically collected as you use these services.

Personal information you provide directly to us includes:

  • Identification and contact information that you give to us, such as your name, member ID, and email address
  • Healthcare claims, forms, and surveys that you submit through the website or mobile app
  • Preferences, such as how you wish to receive communications

Personal information collected automatically through the website or mobile app includes:

  • Device and technical data, such as IP address
  • Page views and access
  • Geographic location data

LifeWise collects only imprecise geographic location data, such as the state you live in. We do not perform background location tracking.

See Cookies and Other Tracking Technologies below for more detailed information about data collected by tracking technologies.

Personal information collected from third parties

LifeWise collects information from third parties, including your employer or health plan, vendors that provide services to your health plan, and healthcare providers. We may display this information in your secure website portal or mobile app.

For example, we collect claims information from healthcare providers and prescription information from LifeWise’s pharmacy benefit provider. We display this information on the website or mobile app so you can view claim status information and prescription history.

As another example, your health plan may contract with a vendor to administer a health or wellness program, such as a health risk assessment. Depending on that program’s terms of use, results from programs or assessments—including body mass index and other health- or lifestyle-related measurements—may appear in your personal health record (PHR) or elsewhere on the website or mobile app.

If you no longer wish to use LifeWise.com or its partner sites, we will endeavor to provide a way to request correction, update, or removal of personal information you provided, subject to applicable retention requirements. To request help or discontinue service, contact LifeWise Customer Service at the number on the back of your member ID card.

Disclosure of personal information

HIPAA, the federal law that addresses health information privacy, governs the disclosure of your personal information. LifeWise and its business associates comply with HIPAA (Health Insurance Portability and Accountability Act) and other applicable privacy laws.

LifeWise may disclose your personal information to business associates or other third parties for treatment, payment, or healthcare operations, or as otherwise permitted or required by law, without your specific authorization. We may disclose personal information to your healthcare provider for the provider’s treatment, payment, or healthcare operations. We may also share personal information with LifeWise affiliates and subsidiaries.

We may also disclose your personal information to your employer or its business associate to administer a wellness program or conduct payment or healthcare operations under HIPAA. For example, if your employer offers an incentive to employees who complete a health risk assessment, we may need to disclose that you completed the assessment so the incentive can be provided. With your consent, we may also disclose your personal information to third parties in connection with wellness programs you choose to participate in, such as to allow a third party to deliver a reward you earned.

LifeWise will not sell your personal information.

For more information on other disclosures that are allowed or required by law, see the LifeWise Notice of Privacy Practices.

How personal information is used

LifeWise or its partners may use your personal information to identify health characteristics and display related information in sections of the website or mobile app. For example, if you complete a health risk assessment and report that you have heart disease, your record may include an identifier for that condition. LifeWise or its partners may use this information with other data to determine whether to provide recommendations intended to improve your health or reduce the risk of an adverse event.

LifeWise or its partners may display identifiers and recommendations in your PHR or send you personalized emails. We may also use information relating to certain health conditions to provide personalized recommendations about follow-up care, lab testing, medications, and other guideline-based treatment options.

We may also send you emails for healthcare-operations purposes, including personalized health information, news, and tips. If you no longer wish to receive these messages, you may opt out by selecting the unsubscribe link in the email or by calling LifeWise Customer Service at the number on the back of your member ID card.

LifeWise or its partners may also send occasional website and service announcements. You cannot opt out of service announcements because they contain important information about the service. LifeWise or its partners may contact you by email or phone to respond to requests or announce significant account changes.

LifeWise or its partners may also use your personal information for business planning and healthcare operations so we can administer your benefits and ensure you receive the highest quality services.

Cookies and other tracking technologies

LifeWise and its service providers may use cookies, device identifiers, web beacons (also known as pixels or tracking tags), log files, and similar tracking technologies on our website and mobile app. These technologies help us understand how users interact with our digital services, maintain and improve functionality, analyze performance, enhance security, and improve the user experience. LifeWise also uses cookies and web beacons (pixels) for outreach, education, and digital advertising. These technologies help us measure advertisement campaign effectiveness and improve the relevance of information provided through those campaigns.

LifeWise does not use tracking technologies to identify users personally unless you voluntarily provide information through your account or as otherwise described in this Policy.

These technologies may collect information such as:

  • Internet protocol (IP) address
  • Device type and operating system
  • Browser type and language settings
  • Pages or screens viewed
  • Date and time of access
  • Time spent on pages or screens
  • Navigation patterns and interactions with website or mobile app features
  • Diagnostic and technical performance information

Third-party vendors that provide analytics or advertising services to LifeWise may collect information about visits to LifeWise websites and mobile apps. These vendors must comply with HIPAA and applicable privacy requirements. LifeWise does not sell personal information collected through its website or mobile app.

Mobile app preferences

You can use your mobile device settings to permit or reject certain communications or types of data collection.

Our mobile app may communicate with you through push notifications. You can enable or disable push notifications through your mobile device settings.

Our mobile app may collect location data from your device to provide services or information related to your location. You may enable or disable location data collection through your mobile device settings.

Security of your personal information

LifeWise takes precautions to protect users’ personal information online and offline, including the following practices:

Encryption: LifeWise uses Secure Sockets Layer (SSL) or similar encryption technology to protect information transmitted through its website and mobile app. A lock icon in your browser indicates that the connection is encrypted.

Authentication methods: LifeWise uses passwords, email multifactor authentication, and/or passkeys to authenticate users before granting access to personal information.

Limiting access to information: LifeWise limits access to personal information to employees who need it to perform their jobs. Third-party vendors receive only the access or information needed to perform their functions. Vendors that are business associates must enter into agreements requiring them to protect personal information in accordance with HIPAA.

Links to other sites: The LifeWise website and partner health sites may link to external sites that provide health information or other services. LifeWise and its partners are not responsible for the availability, content, or privacy practices of those sites and do not endorse, warrant, or guarantee the products, services, or information they offer. When you leave a LifeWise site, review the privacy statement of each external site that collects personal or protected health information. The external site, not LifeWise or its partners, is responsible for complying with its privacy policy. LifeWise presents external material in a way that distinguishes it from LifeWise and partner content.

LifeWise encourages you to report any nonfunctioning links so we can remove or correct those connections. To report nonfunctioning links, call LifeWise Customer Service at the number on the back of your member ID card.

Visiting linked or advertising-service websites: A linked site may collect technical information, record interactions with advertisements, or place cookies on your device. LifeWise does not collect personal information from third-party sites and does not permit those sites to collect other personal information on LifeWise’s behalf without your specific consent.

Protection of children

LifeWise online and mobile app accounts are not intended for children. For members of LifeWise Blue Cross plans in Washington state, we do not collect protected health information from anyone who we know is under age 13. For members of LifeWise Blue Cross Blue Shield of Alaska plans, we do not collect protected health information from anyone who we know is under age 18. A parent or guardian may establish a PHR for a minor and is responsible for supervising the minor’s use of the LifeWise website or mobile app, protecting authentication information, ensuring that submitted information is accurate, and interpreting and using information or suggestions provided through LifeWise for the minor.

Health and wellness: Partner-provided tools

LifeWise provides access to health and wellness websites and mobile apps offered by partners. These tools may include a PHR and other features that help you manage your health and help LifeWise administer your health plan. LifeWise and its partners are committed to protecting the privacy and integrity of your personal information in accordance with HIPAA and other applicable state and federal laws.

Participation in surveys or contests

From time to time, our website or mobile app may request information through surveys or contests. Participation is voluntary, and you may choose whether to provide the requested information. Requested information may include contact and demographic information, such as your name, email address, date of birth, gender, member identification number, mailing address, or ZIP code. We use survey information to improve your experience on our website or mobile app.

How we respond to browser “Do Not Track” signals

Some web browsers offer a “do not track” (DNT) or similar feature that signals a user’s preference to not have online activity tracked. DNT signals are not uniform, and not all browsers offer them. For these reasons, LifeWise, like many website operators, does not respond to DNT signals.

Data retention and deletion policy

We retain personal information only as long as necessary to fulfill the purposes for which it was collected, provide website or mobile app services, comply with legal obligations, and resolve disputes. Website and mobile app data is retained while your account is active or as otherwise needed to provide functionality. Claims, enrollment data, and communications are retained in accordance with applicable laws and other requirements for protected health information.

You may change or delete information you entered and saved in your PHR, including information entered through website tools and assessments. To change or delete this information, go to the PHR section of your online account. If you have difficulty changing or deleting information, call LifeWise Customer Service at the number on the back of your member ID card.

You may delete your mobile app account through the “Delete your account” option in the app or through your online account on LifeWise’s website. Deleting the account will delete mobile app device and technical data but will not alter your claims, enrollment data, communications, or coverage history.

See the Notice of Privacy Practices for information about requesting changes to clinical and coverage records.

Changes to this Digital Privacy Policy

LifeWise reserves the right to revise or amend the terms of its Digital Privacy Policy in accordance with HIPAA and other federal and state law. By using any of our partner websites and the services available, you consent to our collection and use of your personal information as described herein. Any change to the Digital Privacy Policy will be posted to the Policy page on this website or mobile app. The Policy will display the date it was last revised.
